Appointments

Requesting a Consultation... is  a simple process of calling our Receptionist who will try to accommodate your schedule with our appointment availability. Once this is confirmed, you'll be given information on how to easily  access into your own personalized, private & secure, internet based, 'Patient Portal'  part of our 'Computerized Medical Record', in order to provide us with the essential medical information needed before your consultation visit with Dr. Strelzow.

If you cannot keep a scheduled appointment or will be delayed, please call as soon as possible so we can accommodate your change request - as a courtesy.  Charges may be incurred for appointments cancelled less than 24 hours before the scheduled appointment time unless reasonable explanations of the special circumstances are personally communicated to the staff.

Insurances

Our office accepts a wide variety of PPOs, Medicare, and other approved health plans. Please, to avoid confusion - when you call our office for an appointment, always verify acceptance of  your insurance carrier or to make other arrangements to preauthorize your upcoming office visit / procedure.

Specific questions or any concerns about your coverage or billing, i.e.; patient co-pays, Individual plan deductibles, general insurance coverage, usual out of pocket fees, any proceedure costs, etc., should be addressed & clarified with our staff before making your appointment, rather than at the time of or after the office consultation / visit, in order to try to avoid any un-necessary confusion of misunderstanding.
